Testosterone is the primary anabolic hormone in men — governing muscle mass, bone density, fat distribution, libido, energy, mood, and cognitive function. What Drives Testosterone Decline

Leydig Cell Aging

Testosterone is produced primarily by Leydig cells in the testes, in response to luteinizing hormone (LH) from the pituitary. With age, Leydig cell number and responsiveness to LH decline — reducing testosterone output even when LH signaling is adequate.

Elevated Cortisol

Cortisol and testosterone share a precursor (pregnenolone) and have an inverse relationship. Chronic stress chronically elevates cortisol, diverting pregnenolone away from testosterone synthesis. This is the “pregnenolone steal” — stress literally steals the raw material for testosterone production.

Increased Aromatization

Aromatase — an enzyme found primarily in fat tissue — converts testosterone to estrogen. As body fat increases with age, aromatase activity increases, accelerating testosterone-to-estrogen conversion and creating a self-reinforcing cycle of lower testosterone and higher estrogen.

Elevated SHBG

Sex hormone binding globulin (SHBG) binds testosterone in the bloodstream, making it unavailable to tissues. SHBG increases with age, reducing free (bioavailable) testosterone even when total testosterone appears normal on lab tests.
